About OTC allowances
An over-the-counter (OTC) allowance lets plan members buy approved health items, such as pain relievers, vitamins, and first-aid supplies, up to a set amount each month or quarter. Plans deliver it through a card, a catalog, or approved stores.
What to check before you choose
- Unused money often expires at the end of each month or quarter instead of carrying over.
- Each plan sets its own list of approved items and stores.
- Some plans offer the allowance only to members of a Special Needs Plan or to members with qualifying chronic conditions.

Sources and method
- Source: CMS Medicare Advantage plan data, 2026 plan year
Built from the CMS Medicare Advantage landscape, Plan Benefit Package, star rating, and monthly enrollment files. Plan figures are for the 2026 plan year. Enrollment data as of September 2026. Each plan counts once statewide, even when it is sold in many counties. Employer group plans are left out. CMS hides enrollment counts of 10 or fewer, so shares can add up to a little under 100%. Benefit flags follow the plan filings, so a benefit counts whether it is listed on its own or as part of a combined allowance card.
